Many Americans between 45 and 65 years of age experience hemorrhoids. Hemorrhoidal size, thrombosis, and location (i.e., proximal or distal to the dentate line) determine the extent of pain or discomfort. The history and physical examination must assess for risk factors and clinical signs indicating more concerning disease processes. Internal hemorrhoids are traditionally graded from I to IV based on the extent of prolapse. Other factors such as degree of discomfort, bleeding, comorbidities, and patient preference should help determine the order in which treatments are pursued. Medical management (e.g., stool softeners, topical over-the-counter preparations, topical nitroglycerine), dietary modifications (e.g., increased fiber and water intake), and behavioral therapies (sitz baths) are the mainstays of initial therapy. If these are unsuccessful, office-based treatment of grades I to III internal hemorrhoids with rubber band ligation is the preferred next step because it has a lower failure rate than infrared photocoagulation. Open or closed (conventional) excisional hemorrhoidectomy leads to greater surgical success rates but also incurs more pain and a prolonged recovery than office-based procedures; therefore, hemorrhoidectomy should be reserved for recurrent or higher-grade disease. Closed hemorrhoidectomy with diathermic or ultrasonic cutting devices may decrease bleeding and pain. Stapled hemorrhoidopexy elevates grade III or IV hemorrhoids to their normal anatomic position by removing a band of proximal mucosal tissue; however, this procedure has several potential postoperative complications. Hemorrhoidal artery ligation may be useful in grade II or III hemorrhoids because patients may experience less pain and recover more quickly. Excision of thrombosed external hemorrhoids can greatly reduce pain if performed within the first two to three days of symptoms.

A new minimally invasive method for the treatment of bleeding internal hemorrhoids with endoscopic sclerotherapy with Lauromacrogol was investigated, finding no patient had immediate complications and 70 cases showed no symptoms. Background and aim: Inspired by the success of endoscopic sclerotherapy for the treatment of bleeding gastric varices, we investigated a new minimally invasive method for the treatment of bleeding internal hemorrhoids. Patients and methods: From 2009 to 2013, 110 outpatients with 2- or 3-degree bleeding internal hemorrhoids underwent colonoscopic sclerotherapy with Lauromacrogol. Patients with mixed hemorrhoids, fistula and fissure were excluded in this study. All patients were visited after 1 week and followed up through a telephone interview after 1 month, 6 months, 1 years and 2 years. 93 patients were performed colonoscopy examination after 1 year or 2 years. Results: As shown in this study, no patient had immediate complications. Mild postoperative pain occurred in 13 cases (12%) after the initial treatment, but could be alleviated spontaneously without pain medication. 26 patients (23.6%) showed mild self-limiting rectal bleeding one week after the treatment. All patients displayed pelvic infection, rectal prolapse and urinary retentions. Three months later, 95.5% of patients (105/110) were effectively treated. 5 patients carried out another sclerotherapy with injection of Lauromacrogol due to recurrent hemorrhage. A total of 93 patients (85%) were contacted by telephone for a follow-up to document results of the treatment after 1 month, 6 months, 1 year and 2 years. Among them, 70 cases (75%) showed no symptoms. 23 cases (25%) had some residual symptoms including occasional bleeding and prolapse, however, there were no requirements for further treatment. No patients experienced fecal incontinence. The duration of the operation in our study is 16.3±5.1minutes. The total cost of this sclerotherapy with Lauromacrogolis 2536±167.8 Yuan.
